                A Ventila\u00e7\u00e3o N\u00e3o Invasiva (VNI) \u00e9 iniciada nas doen\u00e7as restritivas que n\u00e3o envolvem o par\u00eanquima pulmonar<\/strong> (doen\u00e7as neuromusculares, les\u00f5es vertebro-medulares e deforma\u00e7\u00f5es da caixa tor\u00e1cica) na presen\u00e7a de sintomas de hipoventila\u00e7\u00e3o e de, pelo menos, um dos seguintes crit\u00e9rios fisiopatol\u00f3gicos: PaCO<\/span>2<\/sub>\u00a0<\/span>> 45 mmHg ou dessatura\u00e7\u00e3o noturna, com satura\u00e7\u00e3o perif\u00e9rica de O<\/span>2<\/sub>\u00a0<\/span>(SpO<\/span>2<\/sub>) < 88%, durante 5 minutos consecutivos.<\/p>\n

                Nas les\u00f5es vertebro-medulares<\/strong>, considera-se ainda iniciar a VNI se SpO<\/span>2<\/sub>\u00a0<\/span>< 90% por > 10% do tempo total do sono.<\/p>\n

                Nas doen\u00e7as neuromusculares\u00a0rapidamente progressivas<\/strong>, a VNI \u00e9 iniciada quando a capacidade vital for\u00e7ada (FVC) for < 50% (valor te\u00f3rico) ou a press\u00e3o m\u00e1xima inspirat\u00f3ria (PMI) for < 60 cmHO<\/span>2<\/sub>, independentemente dos crit\u00e9rios anteriores.<\/p>\n

                No caso da Esclerose Lateral Amiotr\u00f3fica (ELA)<\/strong> a VNI dever\u00e1 ser ponderada com FVC \u2264 70%.<\/p>\n

                A VNI est\u00e1 ainda indicada na s\u00edndrome de obesidade\/hipoventila\u00e7\u00e3o<\/strong> com ou sem eventos obstrutivos do sono da via a\u00e9rea superior.<\/p>\n

                Na Doen\u00e7a Pulmonar Obstrutiva Cr\u00f3nica<\/strong> (DPOC)\u00a0<\/strong>a VNI \u00e9 ponderada nos doentes que, apesar de uma Oxigenoterapia de Longa Dura\u00e7\u00e3o (OLD) bem conduzida (quando para al\u00e9m do cumprimento de mais de 15 horas di\u00e1rias de oxigenoterapia, incluindo o per\u00edodo noturno, o doente deixou de fumar, cumpre a terap\u00eautica inalat\u00f3ria e \u00e9 seguido em programas de reabilita\u00e7\u00e3o respirat\u00f3ria), apresentem uma das seguintes situa\u00e7\u00f5es:<\/p>\n

                \u2022 PaCO<\/span>2<\/sub>\u00a0<\/span>> 55 mmHg;<\/p>\n

                \u2022 PaCO<\/span>2<\/sub>\u00a0<\/span>entre 50 e 54 mmHg, numa das seguintes circunst\u00e2ncias: dessatura\u00e7\u00e3o noturna (SpO<\/span>2<\/sub>\u00a0<\/span>< 88% durante 5 minutos consecutivos, n\u00e3o corrigida com administra\u00e7\u00e3o de O<\/span>2<\/sub>\u00a0<\/span>a 2 L\/min) ou mais do que duas exacerba\u00e7\u00f5es agudas por ano acompanhadas de acidose respirat\u00f3ria com necessidade de VNI.<\/p>\n

                A VNI na Fibrose Qu\u00edstica\/Bronquiectasias<\/strong> deve ser considerada desde que apresentem:<\/p>\n

                \u2022 Bronquiectasias difusas;<\/p>\n

                \u2022 PaCO<\/span>2<\/sub>\u00a0<\/span>> 50 mmHg;<\/p>\n

                \u2022 Envolvimento > 3 lobos; FEV1 < 50% do previsto; FEV1\/FVC < 70%; SpO<\/span>2<\/sub>\u00a0<\/span>< 88% durante 5 minutos consecutivos com O<\/span>2<\/sub>\u00a0<\/span>a 2L.<\/p>\n

                A ventila\u00e7\u00e3o por pe\u00e7a bucal (Mouthpiece Ventilation<\/em>) utiliza um circuito aberto recorrendo a uma interface que o doente segura com os l\u00e1bios quando deseja receber apoio durante a inspira\u00e7\u00e3o. Esta pode iniciar-se pela cria\u00e7\u00e3o de uma pequena press\u00e3o negativa no circuito, ou apenas por tocar na pe\u00e7a bucal (exemplo: Kiss Trigger<\/em>). Vocacionada para doentes com grande depend\u00eancia ventilat\u00f3ria cont\u00ednua e empregue, quase que exclusivamente, em doentes com patologia neuromuscular.<\/p>\n

                \u2022 Possibilita alternar uma assist\u00eancia ventilat\u00f3ria diurna por pe\u00e7a bucal com um suporte noturno utilizando m\u00e1scara nasal ou oronasal.<\/p>\n

                \u2022 O modo ventilat\u00f3rio mais comum \u00e9 o controlado por volume (ACV), com volumes correntes elevados, sem PEEP (EPAP), nenhuma ou baixa FR de backup<\/em>, alarme de press\u00e3o baixa ajustado na dura\u00e7\u00e3o m\u00ednima e m\u00e1xima da apneia.<\/p>\n

                \u2022 Pode permitir o aumento dos volumes pulmonares por air-stacking<\/em> (inspira\u00e7\u00f5es sequenciais sem exalar) com o objetivo de melhorar a efic\u00e1cia da tosse.<\/p>\n
